1. Telehealth Appointment Consent
· By scheduling a telehealth appointment with Aleafiate Clinics, you consent to participate in a remote medical consultation with our qualified healthcare practitioners.
· Patients are required to submit a complete Intake Form prior to their appointment, detailing essential medical history and health information. Failure to submit this form in a timely manner may result in appointment cancellation.
2. Understanding Medicinal Cannabis Treatment
· Nature of Medicine: Medicinal cannabis is an unregistered medicine in Australia, with its long-term effects under study. Patients must understand the potential risks and complications, including possible side effects such as sedation, fatigue, and dizziness.
· Legal Restrictions: Driving or operating heavy machinery with THC in your system is illegal across most Australian jurisdictions. A medical prescription does not protect against legal consequences related to THC, and Aleafiate Clinics is not liable for any penalties incurred because of THC presence.
· Medication Interactions: Medicinal cannabis may interact with other medications. Patients must adhere to the dosing recommendations provided by their doctor and promptly report any adverse effects.
3. Follow-Up Consultations
· Patients agree to attend regular follow-up consultations as directed by their healthcare provider. Monitoring progress is essential to ensure safe and effective treatment outcomes.
4. Script Delivery Process
· Post-consultation, prescriptions are sent directly to the patient’s designated pharmacy and to the patient via email, along with required TGA approvals.
· Aleafiate Clinics employs eScript software to identify any potential conflicts with prescriptions from other practitioners. If a conflict is detected, the script may be rejected, and the consultation fee forfeited.
5. Zero-Tolerance Policy
· Aleafiate Clinics has a zero-tolerance policy towards abusive behaviour, including verbal abuse or aggressive remarks directed at staff or healthcare practitioners. Such behaviour may lead to termination of services.
6. Rescheduling Policy
· Patients must provide at least 24 hours’ notice if they need to reschedule an appointment. This notice enables us to offer the slot to other patients in need.
7. Agreement to Terms and Policies
· By proceeding with treatment, patients agree to the latest Aleafiate Terms and Conditions, as well as any applicable Privacy Policies.
